About

About

The Southern Ute Health Center Pharmacy is a full-service outpatient pharmacy located on the east end of the Southern Ute Health Center. We are currently accessible only at the walk-up window. Those with limited mobility may request curbside delivery by calling the pharmacy office at 970-563-4781.

Patients registered at the Southern Ute Health Center with an active and up-to-date chart are eligible to receive prescriptions with no out-of-pocket expense through our pharmacy. Available medications are limited to those in our formulary and by prescription only. Exceptions to our formulary may be made for medical necessity, at the discretion of our Pharmacy and Therapeutics committee, and subject to product availability.

The Southern Ute Health Center Pharmacy is contracted with some insurance plans, including Colorado and New Mexico Medicaid. While our patients are never charged out-of-pocket costs for their prescriptions, we do bill contracted plans, in order to be able to extend our services to all eligible patients.
