The Southern Ute Indian Tribe (SUIT) offers to its enrolled members a program designed to fill coverage for Medical, Prescription, Dental, and Vision covered services past that of coverage under the local Contract Health Services Program.
Medical Program
- The SUIT Medical Program is based in the Southern Ute Health Center and managed by the Tribal Health Department. For services outside of the Health Center, the Tribe has contracted with the Forest County Potawatomi Insurance Network of providers nationally.
- For those members who utilize services outside of the Forest County Potawatomi Insurance Network, there is a co-pay and deductible that applies to services that the member will be responsible for.

Dental Program
- The SUIT Dental Program is based in the Southern Ute Health Center and is managed by the Tribal Health Department. For services outside of the Health Center, the Tribe has contracted with the Delta Dental Network of providers nationally.
- For those members who utilize services outside of the Delta Dental Network, there is a co-pay and deductible that applies to services that the member will be responsible for.

Vision Program
- The SUIT Vision Program is based in the Southern Ute Health Center and managed by the Tribal Health Department. For services outside of the Health Center, the Tribe has contracted with the VSP Network of providers nationally.
- For those members who utilize services outside of the VSP Network, there is a co-pay and deductible that applies to services that the member will be responsible for.

- The Health Center can get the Glasses under prescription at no cost to you. Please contact the Health Center Vision Department for more details.
Prescription Program
- The SUIT Prescription Program is based in the Southern Ute Health Center and managed by the Tribal Health Department. For services outside of the Health Center, the Tribe has contracted with the SilverbackRx Pharmacy Network.
- For those members who utilize services outside of the SilverbackRx Pharmacy Network, there is a co-pay and deductible that applies to services that the member will be responsible for.

- Mail order prescriptions: Those drugs that are identified as high cost, maintenance drugs will be filled under the SilverbackRx Mail Order Prescription Plan.
